1. Authority and accuracy. I declare that I am the person named in this request, that I am authorised by the organisation named to make it, and that all information and documents I have supplied are true, complete and not misleading.

2. Lawful purpose. I am requesting this verification only for the lawful purpose stated. I have a lawful basis, including any consent or legal authority required under the Personal Data Protection Law of the Kingdom of Saudi Arabia and other applicable law, to receive and process any personal data in the resulting report.

3. Confidentiality and use. I will keep any report and the details in it confidential, use them only for the stated purpose, and not copy, publish or disclose them to anyone who is not entitled to them, except where the law requires.

4. Nature of the report. I understand that an Official Verification Report states only what the records of Jadwal Al-Barakah Company showed at the date and time it was issued. It is not a legal opinion, guarantee or certification of any other fact, and a document may later expire, be suspended or be revoked.

5. Limitation of liability. To the fullest extent permitted by law, Jadwal Al-Barakah Company, its owners, officers and employees accept no liability for any loss or damage arising from my or my organisation’s reliance on, use of, or decisions made on the basis of a quick check or a report, or from any delay, refusal or error in providing it, except where liability cannot be excluded by law.

6. Records, checks and sharing. I consent to Jadwal Al-Barakah Company recording this request, including my details, identity document details, uploaded files, IP address and device information; verifying them with me, my supervisor or my organisation; keeping them for audit and legal purposes; and sharing them with competent authorities where required by law.

7. Misrepresentation. I understand that false or misleading information or documents may lead to refusal or withdrawal of a report, may be reported to the competent authorities, and may expose me and my organisation to legal liability.

8. Electronic signature and governing law. Typing my name below is my electronic signature and has the same effect as signing by hand, to the extent permitted by law. This declaration is governed by the laws of the Kingdom of Saudi Arabia.
